NDIS Registered Support Coordination

At Create Disability Services, we provide independent, structured Support Coordination designed to help you understand, implement and make the most of your NDIS plan.

We work alongside you to translate your funding into practical supports, linking you with community, mainstream and government services while ensuring your plan is implemented clearly and effectively.

LEVEL 1

Support Connection

This level of support helps you connect with informal, community and funded services so you can begin implementing your NDIS plan with confidence.

We guide you through the early stages of understanding your plan, identifying suitable supports, and building the foundations needed to move toward your goals.

LEVEL 2

Coordination of Supports

Coordination of Supports assists you to understand, implement and actively use your NDIS plan.

We work alongside you to coordinate a mix of supports that strengthen your capacity to:

• Maintain relationships
• Manage service delivery tasks
• Increase independence
• Participate in your community

Our role is to ensure your supports are aligned, responsive and working effectively together.

LEVEL 3

Specialist Coordination of Supports

Specialist Support Coordination is designed for participants with more complex or layered circumstances.

This level of support provides structured oversight and collaboration across multiple services and systems. We assist to:

• Address barriers impacting access to supports
• Stabilise and coordinate complex service environments
• Navigate safeguarding and risk-sensitive matters
• Develop structured service planning where required

Our approach is calm, strategic and future-focused, ensuring consistency and accountability in complex situations.

 

Self-Management Capacity Building

If you are self-managing your plan but would benefit from guidance, you may be able to use part of your Core funding to build your confidence and skills in managing your supports.

We provide practical assistance to help you understand budgets, service agreements and provider expectations, so you feel informed and in control.
