NDIS Registered Support Coordination Services

As Support Coordinators within Create Disability Services, we will support you to understand and implement the funded supports in your NDIS plan and link you to community, mainstream and other government services. We focus on supporting you to build skills and self-direct your life, as well as connecting you to providers.

Our team will assist you to negotiate with providers regarding what they will offer you and how much it will cost. We will ensure that your service agreements and service bookings are completed and will build your capacity to exercise choice and control, coordinate supports and assist you to access your local community.

As Support Coordinators we can also assist you to plan ahead and prepare for your plan review and will assist you to ‘optimise’ your plan, ensuring that you are getting the absolute most out of your funded supports.

LEVEL 1

Support Connection

This support builds your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.

LEVEL 2

Coordination of Supports

This support will assist you to build the skills you need to understand, implement and use your plan. We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

LEVEL 3

Specialist Coordination of Supports

This is a higher level of support coordination for people whose situations are more complex and require specialist support. We will assist you to manage challenges in your support environment and ensure consistent delivery of service, address any complex barriers that affect your ability to access the right supports, and, design a service plan for your specific support needs, where needed.

 

Self-Management Capacity Building

If you are Self Managing your plan but feel that you need a little help navigating NDIS-land, you can use part of your Core funding for ‘Self Management Capacity Building‘.

Connecting with Create Disability Services

It is a new, slightly scary, but exciting time in NDIS-land at the moment and we can help you embrace all of the opportunities that are out there, while side-stepping or minimizing the complications.

As you can see by these descriptions for each type of NDIS support coordination, the role of a Support Coordinator can be varied and requires skills in relationship building, crisis and issues management experience and a thorough understanding of the NDIS.

At the end of the day, it is our role as Support Coordinator’s is to help you achieve your goals and at Create Disability Services we will do everything in our power to get you there!

Connection

Connecting participants with service providers within the community, a Support Coordinator plays a critical role in the ongoing success of plan implementation. Because of the knowledge and connections that Support Coordinator’s have with service providers, they’re able to make referrals that best align with their clients’ needs.

Plan knowledge

Many people struggle to understand every aspect of their NDIS plan or if you’re new to the NDIS, understanding terminology can take some time. This is where a Support Coordinator comes in! Their knowledge of the NDIS and supports is invaluable. It’s also important to know how your plan will affect you so a Support Coordinator will take the time to understand your needs and make appropriate recommendations.

Provide choice

A Support Coordinator’s role is to show all available and practical options to their clients to ensure they have choice and remain in control of their supports. They will also provide their recommendations based on what options will be best for each individual’s circumstances to assist in decision-making.

Crisis resolution

If any issues were to arise with service providers, a Support Coordinator is there to help resolve any of these issues or act as a mediator if required. They can also end any provider relationships if the fit isn’t right.

Reflect and refine

Once you’ve settled into your plan, your Support Coordinator will work with you to reflect on your experiences to date and make any adjustments necessary (where possible) to ensure the ongoing success of your plan.
